Each child enrolling in Petite Artistes classes must have a Participation Agreement filled out for them by a parent or legal guardian.  Click the link below for a downloadable PDF copy of this agreement.  No child will be registered or enrolled in class without this form and full payment.  You must have Adobe Acrobat Reader to print or read this file.
